Hurricane Harvey Medication Relief Program
Purpose: HarborPath has implemented a short-term medication program to assist patients displaced by the storm in southeast Houston and surrounding areas and Louisiana with their HIV medication needs.
Patients must be uninsured*
HarborPath application signed by patient and provider
A signed statement by the patient that he/she has been displaced by the storm or letter from Texas ADAP office
This program is available to new or existing patients. Medications will be shipped via ups overnight delivery to a patient's home* if able to receive deliveries, pharmacies, clinics.
The eligibility period will be for 30 days and re-enrollment will be considered on a case-by-case basis.
*Uninsured is defined as not having commercial insurance, Medicaid, Medicare, and not being served by the state ADAP program.
*Medications can be shipped to an alternate address if the patient is displaced and staying with relatives, friends, etc.
